Transforming inpatient care - emergency admissions - key worker alerts - SWBH
Aim:
To reduce the average length of stay following an emergency admission for known cancer patients by 30%. To reduce readmission rates to 18% at Sandwell and West Birmingham Hospitals NHS Trust (SWBH). To aviod admissions where possible. The following tumour sites have been identified; colorectal, upper GI, lung, urology and gynaecology.
Objectives:- Use in-house IT systems to notify the key worker of an emergency admittance.
- Ensure the key worker is empowered to facilitate the patients journey.
- Capture reasons for admittance and discharge delay to identify opportunities to address these issues in primary care and acute care.

Aim:
To reduce the average length of stay following an emergency admission for by 30%. To reduce readmission rates to 18% at Walsall Healthcare NHS Trust (WHT). All tumour sites will be included.
Objectives:- Use in-house IT systems to notify the key worker of an emergency admittance.
- Use in-house IT systems to populate white boards with alert status and provide links to policies and protocols.
- Ensure the key worker is empowered to facilitate the patients journey.
- Capture reasons for admittance and discharge delay to identify opportunities to address these issues in primary care and acute care.
