Haematology cancer patient self assessment
The haematology cancer patient self assessment is based on the assessment document piloted during the national cancer survivorship initiative (NCSI). The document uses a series of questions to be answered by patients and their careers to highlight issues that go beyond the medical effects of cancer and treatment. The answers are used to inform a discussion between the patient and their haematology clinical nurse pecialist (CNS) in order to identify and agree a care plan that addresses the issues raised by patients.

Advice sheet for haematology cancer patients
The advice sheet compliments the self assessment. Both documents focus on five domains; treatment, physical concerns, emotional concerns, family concerns and money worries. The advice sheet provides both advice and a list of local services for each of the domains. Patients are encouraged to contact and make use of the services available to help with the issues highlighted in the self assessment.

The useful contacts for CNS document was created as a resource for the Haematology CNS. It was designed as a single document to replace the multiple locations that contacts are often stored. The layout and style matches that used in the advice sheet for patients to enable quick reference to specific issues. The document is A3 in size, it can be laminated and can be kept in multiple locations.
